Board Approval Form
The School
Board meeting dates together with the dates the agenda materials are due to the
Special Projects Office can be found here for the
current school year.
School based personnel should include a cover memo from their
principal. This process ensures the principal is aware and approves of the
grant application.

It is used to add an
employee to the PCSB payroll for a specified period of time. Once approved and
work is completed, a Payroll Adjustment Form (see below) must be completed so
a check can be issued.
Complete this form to compensate employees for travel outside Pinellas County.
Expenses may include taxi fare, registration, vehicle rental, tolls, lodging
and communications. Meals are included at the current per diem or per meal rate.
Complete this form to compensate with stipends School Board employees who
officially are requested to participate in in-service education or professional
development workshops that meet outside of normal duty hours.
Complete this form to determine eligibility of an outside contractor to be
classified and hired as an independent contractor, entered as a TERMS vendor
(following completion of a W-9 form) and paid through an invoice.
Complete this for any employee who will be working at an alternative location
as a School Board employee. It is required whether or not a substitute is needed.
Complete this form for eligible independent
contractors to be entered into TERMS as a vendor. This must be done prior to
completing work and submitting invoices.
