Note: If Pinellas County Schools change insurance providers in the future, new documentation may be required for approval to be on that company's plan.

• EyeMed "Know Before You Go" out-of-pocket estimator - Members can select any optometrist or ophthalmologist in the Eye Med Vision Care Advantage network.  At the time of your appointment, you will pay the applicable copays(s) for your exam and your eyeglasses or contacts, plus the co-pay(s) for any extra covered option(s) you select.

There are no forms to complete or claims to file when you use an EyeMed In-Network Provider.
