To: Parents or Guardians of all 6th Grade Students
Your child will not be permitted to enter or attend the 7th grade in August 2023 without proof of the following immunizations:
- One Tetanus-diphtheria-pertussis (Tdap) booster
For the school year 2023-2024, students entering or attending 7th grade are REQUIRED to have a Tdap (tetanus-diphtheria-pertussis) prior to attending class on the first day of school or valid documented exemption. This grade level is also required to have two varicella vaccines.
All schools in the county are required by Chapter 64D-3.046, Florida Administrative Code, to enforce these immunization requirements.
A completed Certificate of Immunization form (DH 680) is issued when the student has received all required immunizations. After your child has received these vaccines, please turn in this completed form to your child’s school. A receipt from Walgreens, CVS, Minute Clinic or other places are not accepted. MUST be on a FLORIDA 680 Form.
