    Online Forms
    All students are required to have a field trip form signed by a parent prior to attending a field trip. Although specific information regarding the field trip location is not filled out on this sheet, the classroom teacher can update this at school if you choose to use this form. Keep in mind that if your child usually buys lunch at school you will need to inform the teacher if your child will need a lunch for the field trip or if you intend to pack a lunch.
    You can download the Adopt A Class form to make a donation to your child's classroom or teacher at Oakhurst.

    All NEW school volunteers must complete a registration form online for submittal to the District office.  The site is www.pcsb.org/volunteerregistration.  For more information, please visit New Volunteer Information.

    All RETURNING volunteers must re-activate their profile in the volunteer Focus system by logging on to https://focus.pcsb.org/volunteer.  For more information, please visit Returning Volunteers Reactivation Link. (This must be done at the beginning of each school year.)
