Transcript Request Form Copies of the Transcript Request Form are available at the front desk to request your final transcripts are sent to your post-secondary choice. Most in-state transcripts are sent electronically and will need the campus you are attending. Paper transcripts will need the complete mailing address of the school location. The completed form should be sent to Mrs. Parcel at email@example.com for her to process.
Unofficial transcripts which are sent to colleges during the application process are available using the same process stated above.
