• Before tryouts, all students must have the following:

    • Current school year physical - Students can go to their own doctors or to walk-in clinics for physicals.  This is not the FHSAA form, it is just a regular physical form.
    • Current school year participation form - Participation Form or find school participation forms in all grade level offices, the front office, or from a coach.
    • Current school year accident insurance. More information and forms here
