 Athletic Clearances

To do checklist:

1) Print the EL2 form and UPLOAD the form into Step # 1 in AthleticClearance.com

2) Print a copy of your HSR Insurance proof of purchase and UPLOAD the form in Step # 1 AthleticClearance.com

3) Print a copy of your concussion certificate and UPLOAD the form in Step # 1 in AthleticClearance.com

4) UPLOAD the notarized participation form on Step # 1 in AthleticClearance.com under Other Forms.

***ALL FORMS MUST BE UPLOADED TO HAVE A COMPLETE REGISTRATION PACKET

**Special Note**

Uploaded documents to include the EL2 (physical) are acceptable provided the physical includes the official stamp, date, and signature from the physician that completed the physical.  Forms without the official stamp, current date, box checked for Cleared without Limitation, and physician's signature will not be accepted.
