    ATHLETIC PAPERWORK INFORMATION

    Students who would like to participate in athletics must complete the following paperwork before they are permitted to workout, condition, tryout,  or practice for any of the athletic teams:

     

    Florida High School Athletic Association Clearance for Participation Form---GA7

     

    Pinellas County Schools Application for Athletic Participation

     

    Relevant Information Regarding Extracurricular Athletics Form

     

    High School Activities Participation Form (must be notarized)

     

    Pre-Participation Physical Evaluation Form (FHSAA-EL2 form)

     

    FHSAA EL3 Form  (four pages...must have parent and student athlete signatures on each page)
     

    Addendum to High School Activities Participation Form

     

    Student insurance is required to participate in sports. 

     

    Students participating in athletics for the first time, must submit an original birth certificate to the athletic director.

     

    Student athletes must complete the following courses before they can participate in athletics.

    The courses can be found on the NFHS website

    The three courses are as follows:

    1. Concussion in Sports

    2. Heat Illness Prevention

    3. Sudden Cardiac Arrest

    Student athletes must print and turn in a completion cerfiticate for each course.

    *Students in all grade levels must maintain a cumulative grade point average of 2.0 in order to participate in sports.

    A ninth or tenth grader who does not have a cumulative 2.0 grade point average may sign an academic contract and participate in athletics, provided he/she has a 2.0 grade point average the previous semester.
