    Periodic eye exams are an important part of routine preventive health care.  Early detection and treatment is critical for maintaining good vision and preventing permanent vision loss.  Eye exams can detect symptoms of diseases such as diabetes, hypertension, glaucoma, cataracts, and macular degeneration.

    EyeMed "Know Before You Go" out-of-pocket estimator Tool - Members can select any optometrist or ophthalmologist in the Eye Med Vision Care Advantage network.  At the time of your appointment, you will pay the applicable co-pays(s) for your exam and your eyeglasses or contacts, plus the co-pay(s) for any extra covered option(s) you select.

     

    There are no forms to complete or claims to file when you use an EyeMed In-Network Provider.
